West Midlands Care operator chooses UK hosted Online forms solution
After using an American based Online form provider for a number of years, Dove searched to find a UK based alternative solution for a number of reasons including UK data hosting, product support available during the same working hours as themselves and competitive pricing.

The Challenge
With over 30 different Online forms being used daily by client facing care workers plus office staff the initial focus was to transfer the forms and their on-screen layouts if possible to the pro-Forms® platform. Due to their current provider contractual agreement ending in a matter of a few days, time was tight to avoid any interruption in the capture of key client data by staff.
In the event all 30 forms were recreated by hand using the rapid pro-Forms® drag & drop interface for adding form questions and controls.
A Dove branded theme was created, saved and applied to all the forms ensuring familiarity for staff and external people submitting forms such as job applicants, family/ relatives and people providing Character References.
The outcome
With the tight time scale successfully met, Dove staff were able to seamlessly swap from their previous Jotform platform to pro-Forms® without any down time or interruption to the care services provided. On launch day a suite of both admin, HR and care forms were available to staff including:
- DBS Information request
- Care package initial assessment
- HMRC New Starter Check list
- GDPR consent forms
- Medical assessment
- Moving & handling risk assessment
- Service User Plan of Care
- Service User Plan of Care
- Live-in Client daily routine
Averaging between 25 and 30 submitted forms each day, admin time has become manageable thanks to each form being able to be routed to either different email addresses, cloud storage solutions such as OneDrive or other destinations including SharePoint.
Using the pro-Forms® Conditional Triggers feature also gives Dove to intelligently route forms to different people, systems or locations based on answers given during the form completions and submission process.
